Thank you for visiting our store. This page is for companies that require a collection from their store. If you are local we may be able to come out to you for in store LCD testing.

Once this form is submitted a courier will be instructed to collect your LCDs and you will receive your packaging slip via email. Please read our packaging instructions guide. If you are local to us a company representative will be in contact shortly after submitting this form.
